AFFIDAVIT OF NAME CORRECTION

I, Md. Abul Hasan, Father: Md. Jomir Uddin, Mother: Ambia Khatun, Address: Binodpur, Panchashar, Munshiganj Sadar, Munshiganj, Religion: Islam, and Nationality: Bangladeshi by birth, do hereby solemnly affirm and declare as follows:

01.        That I am a permanent resident and citizen of Bangladesh by birth.

02.        That, my name “Md. Abul Hossain” has been wrongly written in NID No.-8203615656,  Purbali Bank Limited Account No.-3279101067750.  But my correct name is  “Md. Abul Hasan”  which has been correctly written in my passport no.-EE0805209 and Purbali Bank Limited Account No.-3279101071444.

03.        I am declaring that “Md. Abul Hossain” is my wrong name and  “Md. Abul Hasan”  is my correct name.   

04.        I am also declaring that the name of “Md. Abul Hossain” and “Md. Abul Hasan” are meant only to me. So, the two names like “Md. Abul Hossain” and “Md. Abul Hasan” will be treated as same person.
